私は、1978年の卒業後約10年間に論文を1枚も書きませんでした。その理由は、「論文を書き、業績を積むことが出世の道である」ということがとてもいやだからでした。その論文至上主義が日本の卒後の臨床教育の阻害因子になっていると考えていました。医師は、患者を適切に管理でき、助言できるということが必須であり、それができないのになぜ論文を書かなければいけないのか、論文を書く人はむしろ臨床をできない人だと思っていました。

1986年に英国へ短期ではありましたが、留学する機会がありました。そこで、むこうの人は私によく「おまえの専門は何だ」と聞かれました。当時の私は論文を書いていないことを自負していましたので、「臨床をしている」と言うと、「それはあたりまえだ。それで何をしている？」と聞かれました。彼らは、多くの臨床研究をしていましたが、臨床をきっちりとするのは当たり前という感じでした。また、「おまえはいままで何をしていたのか、書いた論文をみせてくれ」といわれ、何もないので困ったことをおぼえています。

以来、興味深く示唆に富む症例はできるだけに論文にしようと、この10年余りやってきました。論文を書き出して、わかったことは、

論文を書くために多くの関連論文をよむため、自分の知識を整理できたこと
他人の論文の方法論を評価することで自分が考えた臨床研究の方法論についても、より科学的に考えられるようになったこと

でした。論文を書くことは、他人（大学）のためではなく、業績のためだけではなく、自分のためなのです。
